Associates, PC, appreciates your cooperation in following the policy outlined
below for scheduling an appointment to present your product information to our office staff. We kindly
ask that you do not stop at the clinics unexpectedly.

Appointments are available at both office locations as below:
-
Methodist - Tuesday,
Wednesday, and Friday ~ (approximately 15 - 20 employees)
-
West Dodge Medical Plaza
- Monday and Wednesday ~ (approximately 5-10 employees)

Presentation to the office staff will be
in our office lunch room, between 11:30 am and 1 pm. You are
requested to complete your presentation and leave by 1:00 pm to allow other staff members, who have elected not
to attend the presentation, to have access to the break room. |

Due to our limited space, please limit your
presentation to one pharmaceutical representative. If more than one representative
wishes to attend, indicate who the representative is and why their attendance
is requested. |

Bring your product information with you.
Abstracts and journal articles of published information are preferred.
Also have information on product pricing and insurance reimbursements. |

Please plan to provide any sample products or
gifts for the patients at the time of your lunch appointment. We will contact you
at other times if we need more sample products. Due to the disruption of our staff
and patients, unscheduled visits to our office for any reason are discouraged. |

We appreciate having lunch provided for the office
staff. Please plan on providing for the approximate number of employees shown for your
requested clinic, although this number is subject to change on any given day. Our
physicians will make every effort to attend your presentation during the designated
time. |

If you are unable to keep your appointment date and
time for lunch, please notify us as soon as possible. |
8. |
|
If you are scheduling for a 'drop off' appointment,
please let us know what day you would like to schedule as well as whether you would like
a morning or afternoon time frame. Please have your literature, product samples for the
office, and/or gifts for our patients ready to give to the front desk associate when you
come in. |
